Let me just shoot in the blue, as I haven't seen one of the videos: did you have portrait pictures in the background, may be of a different person?

Was your videos removed after being accepted or is it that they were not accepted at all?

The model releases: You may have different dates for model signature, your signature and witness signature. All signature should be done at the same time, hence the same date. Otherwise you need to double check to see what you might have missed. Alternately, you can use the electronic model release which is much easier and requires no witness. You'll need the models' email addresses.
