ファイルダウンロード：個人情報入力をさせて、その情報を残す方法

This is a 3 step process:

1. Am HTML contact form to collect data,
2. a secure script to process data and send it to a destination such as an e-mail account or database,  
3. a script that forces the file to download.

・ホームページ上で、ファイル名をクリックすると、「保存しますか？、開きますか？」と応答するようになってます。

※誰がダウンロードしたかなど、全く分からず、誰でもダウンロードできる状態になってます。

【希望する状態】

　・ホームページ上で、ファイル名をクリックすると、「個人情報を入力させる」→「保存しますか？、開きますか？」と応答するよう　　

　にしたい。

　※個人情報を入力した人のみダウンロードできるようにしたい。

I do not understand your question.  Please re-phrase it better.

You speak of downloading a file on the home page.  What home page is that? What file are you downloading?
