    Hello BigArtK,

Sorry for the delayed response and inconvenience caused. Export of a PDF form into Excel can be achieved by custom paid JavaScript as mentioned by Gilad D.

You can also subscribe to Acrobat Pro DC subscription to export PDF forms into Excel. For plan and pricing, you can refer to Adobe article Plans and pricing | Adobe Acrobat DC

For Collecting and managing PDF form data, you can refer to Collecting and managing PDF form data, Adobe Acrobat

Feel free to update this discussion for any further assistance.

    Regards,

    Anand Sri.

In principle yes, but the form data can also be extracted using a script, and then copied into an Excel spreadsheet.

For example, this (paid-for) tool I've developed will do it: Custom-made Adobe Scripts: Adobe Reader -- Import/Export PDF Form Data

    Thank you.

For the small amount of data that will be needed we will just transcribe from the hard copy.
