Captivate 2019: Inviting feedback at the end of a module

We have eLearning modules with software simulation and some with quizzes as well.

At the end of the module is it possible to solicit user feedback by maybe including a small form with about 2 to 3 questions, where the user submits it?

    Depends how you want to get the results, and on the features of your LMS (I suppose you use a LMS).

    Here are some possibilities:

    • Use Short answer questions. 
    • Use a Fill in the Blank question, where you can have multiple blanks, not so good if you expect long answers.
    • Create a form with multiple Text Entry Boxes.
    • Create a form with multiple Scrolling Text Interactions.
    • Attach a pdf which has a form, especially if you have access to Acrobat Pro. Transfer of the results could be more difficult.
