Unable to hear audio passed audio channel 3 and all audio is coming out in mono

Re the screenshot of Audio Channels, this shows that you only have audio on channel 1 and 2. So regarding the title of your thread, you wouldn't get audio from this clip for more than 2 channels.

What do you get when you drag the clip to that sequence?

so are all these settings correct ? 

You cannot reset the audio.

Have to start an new sequence and copy content over.

Best is to start over and set the sequence audio channel format to stereo.

Also the field option should be Progressieve (unless you are shooting interlaced but I doubt that.
