PDF replaces text with boxes when saving as form

We received PDFs from another company to use for applications to them. To streamline the process on our end, we add form fields so we have a reusable version we can send to clients to sign. For some reason, this particular PDF is replacing regular, unedited text with boxes when we save the file after adding the form. At no point do we access the Edit PDF tool, just the Prepare Form. I've attached the first page of the form as received (which we were able to pull the first page from and save without encountering errors) and the page after adding form fields and saving. I'm at a loss because these files are received as PDFs so I don't understand why adding form fields would break it like this.

 

If possible, I'd like to understand why this happening and how to stop it in the future, or a workaround if that's not possible. This is a 42-page application and I don't want to go through every page fixing the text if at all possible.

I have checked both forms. The sample 1 form shows that form fields are already added to the document. And no text information is added in the fields. 

The sample 2 forms seem to be the ones with filled-in information. If that is correct, there appears to be an issue with the font. This generally happens when the font is not embedded in the document and on the machine.

However, without checking the original documents, we cannot comment much.
