Picture Selection Problem AND layered order

I have created a Custom Character sheet for Pathfinder. 
On the first page is two places to put pictures. One picture is a Portrait of the charcter, the second is of an Animal Companion/Pet. For the life of me, I can NOT get it to put two separate pictures. It keeps defaulting to using the one image. NOW for the second problem. On the 5th page there is a page to help players create a Character Card. A small playing card sizeed image of their Character and their Companion/Pet. That page has a few layers for the picture. The page has a static JPG to work from visually. The IMAGE a person loads on the first page is supposed to show on the comparable 5th page BUT, it will have a static image on top of the selected image AND THEN some form fields that will automatically fill in that will be on top of all that.

I have put a 6th page in the file to SHOW that it works from an older file I did this with. However, for the life of me, I can't figure out where I went wrong here. For example, on PAGE 5, I can have the elements layer properly UNTIL they are lined up. THEN the picture jumps to the front. BUT on page 6, this does not happen, yet I don't know why.

Does anyone have a suggestion.
I have included the PDF Form here for anyone to poke at.
Page 6 is here in the file as a sample from an old file that seemed to work, but in the final version, Page 6 will go away. This will be a 5 page file document when done.

I am looking forward to finding out what I am doing wrong.

Spec Info:
Mac using OS 10.11.6  (Early 2008 Mac Pro) ((nope, upgrading is no longer an option. The hardware won't go any further and no money to buy a new computer)
Software being used: Adobe Acrobat X Pro.

The graphics have different names.
One is called "Portrait" and the other "Landscape"

Issue #1 was addressed above.

Re issue #2: You should not create overlapping fields if you intend to show them at the same time, since you can't explicitly define their z-order (ie. which one appears on top and on which one on the bottom). Basically, the last field you modify will be on top, but that's not a sustainable solution. You need to find some other way of doing it.
