adobe acrobat pro "box highlighter"

i am currently using adobe acrobat pro.

I want make my PDF look like "After 2" but I don't know how to do this.

i think it will fist go from "After 1" and then "After 2".

I don't know that is this a highlighter or any other thing.

I really need this. Please guide me about this tool or trick.

You can design the form in Adobe InDesign and export as PDF.
