Alternate text seems twice in Acrobat DC

Hi,

 

I used "Edit Alternate Text" (see, pic 1), and entered some text in the "alt-text dialog box" (see, pic 2). Then I checked to right-click on "figure tags" and it seemed twice (see, pic 3). why?

I hope you already have found a solution to this issue. 
However, I tried testing the scenario on my end and could not reproduce the issue. Was the auto-tagging done already on this file? Is it possible for another application to create this file?
