can't change date format for user to enter e.g. DOB it enters today date

Hi 

I'm still having issues creating an online induction where previously I could automatically insert the new employee's name as it would pick it up from the email name, but in the past couple of months the format has changed.

I can't add a date where the signer can add their DOB it only enters todays date.

It won't let me change to switch to new authoring format and no longer allow me to enter person name with the email address as per below.
