Can we make a PDF form and attach a file on this form

As you have mentioned above, by "to make a PDF form and attach a file to this form" do you meant to create a field to attach a document?

Or you are referring that you need to attach a document to the PDF form?

To attach a file to the PDF, you may refer to the steps mentioned here Links and attachments in PDFs, Adobe Acrobat

If you are referring to create a field to attach PDF in the form, by default there is no option for that.
