Check boxes not working properly

I've created an evaluation form and the check boxes are not working correctly. Can someone help me make adjustments to fix the problem?

"How do I change the name of rows?"

It's easier and faster to delete and recreate these fields, there are tricks that allow you to do this in less than a minute. Attached.

Multiple issues there:

- First column are radio-buttons and the others are check-boxes. Convert them all to the latter.

- Did you mean to create an Adobe Sign form? If not, revert it back to an Acrobat form.

- Give all the fields in each row the same name but unique export values (preferably 1-5) so they act as a single group.

Thank you! I didn't even realize I chose different types of boxes. I was trying to create a form that could be signed by participants but I don't think I didn that correctly either. How do I change the name of rows?? I didn't even know they were named. I'm very new to this. It was the first fillable form I've ever made. Thanks again for your help!!

To revert the form click on Tools - Prepare Form and then click on "More" at the right side of the window and then on "Revert to Acrobat Form". In the future, do not tick the box that says "This document requires signatures" when creating the form.

 

Once you did that delete the column with the radio-buttons and copy one of the others in its place.

 

To rename the fields right-click them (in Prepare Form mode, of course) and you'll see a Rename Field command. Make sure each row has the same name so they act as a group.

I'm afraid you'll have to do it one field at a time.
