Check Boxes that have text

Hello all,

I'm hoping someone can help me solve this mystery 🙂

I have a form that has many check boxes. When selected an "X" will appear in the box. However, we are finding somewhere along the way, text is there instead of the X. It seems to be in the same place of the document - bottm left - "backing" No backing". I've attached the doc and examples of what is happening. 

 

Here are the steps that the document goes through...

The Designer should mark the correct boxes, then has the client sign in-person or will send out to be e-signed. Once the document is signed the designer flattens the doc to combine it with other documents. At some point during these steps. The X that should be in the box turns into text. 

 

Any help, thoughts, ideas is appreciated. Thank you!

Are the screen shots from different PDF viewers? Or are all of them from Acrobat/Reader?

They are from different viewers. Should all be from Acrobat Standard

The fields are standard radio buttons with the default export values.  There is no way that Acrobat Pro, Standard, or Reader, or most other PDF viewers would display "NO" and "YES" in the radio button field.  This text must be from a different viewer. Please list viewers that were used.

Please share the actual file. It's very hard to know what's going on based only on screenshots.
