How does one draw with the pen under the text layer of a PDF (to e.g. manually highlight text, draw a watermark, etc.)?

Well, at least a variation seems possible (https://forums.adobe.com/message/9802087#9802087​), but the "hold down Ctrl" trick doesn't work on my Mac, nor do any of the other key-combinations I've tried. Does anyone know why that is?
