How can I create a PDF form that locks customizable fields and be filled later for submission

I have been asked to create forms such as the one I attached.  I want to know how and what is the best way to do this.  I initially created a form in InDesign and tried working with interactive PDF forms but can't seem to create the lock button (logo) for the customizable fields. The first screenshot shows all the active fields. The second screenshot shows the fields that are left unlocked to have individuals invited fill in their information.  I was told initially this was created with Distiller, but now I'm not sure how since it's been discontinued.

No, Distiller has not be discontinued.

Any luck im having the same problem I want the header to be noneditable but the field to be editable
