How do I print a booklet from a PDF with two-up pages?

Question

Sometimes I receive a PDF pre-formatted for booklet printing but I can’t print it as a properly formatted booklet.

The pages in the PDF are landscape with two booklet pages side by side upright on each landscape page, but booklet page 1 is on its own upright in the middle of the PDF’s first landscape page, and there might be a final booklet page also upright in the middle of its own landscape page. If I select booklet when printing the PDF the first and last booklet pages appear to print OK A5 size on the correct side of the A4 sheet but the printed A5 booklet pages between each contain one landscape PDF page laid across it, the two booklet pages within it being reduced to A7 size.

I have tried playing about with the print options portrait, landscape, auto-rotate but they don’t help.

Attached is an example (edited - the original contained some pages which were double-spreads across the whole landscape page).

Any advice?

I've developed a (paid-for) tool exactly for this purpose, it splits and re-sorts booklets and converts them to "normal" files.
