How to add an field information/description on Text Boxes

I have a PDF form with several input fields.

What I want is when the user is in a specific input field, it would display in the input field or somewhere in the screen an information about the field.

Example:

Name     [  Last Name, First Name MI.   ]

guiding the user on how or what information should be entered.

Is this possible in Acrobat DC?
