Download the free abracadabraTools for Acrobat Pro and install the "New" plugin:

https://www.abracadabrapdf.net/utilitaires/utilities-in-english/abracadabratools_en/

With a single click you can create new PDF documents by choosing from around forty standard formats.

Yep...
seems to be an issue Adobe is not commited to fix, with third party tools still being suggested.
Is it really so hard to maintain a tool that meets the requirements of a user that is not from the US??

Still nothing on this Adobe. Shame! :).

Anyway for those who hate wasting time on this silly omission...just make an blank A4 in indesign, save it as a PDF and voila, done you have an A4 or whatever size you want template for you to open and use to make forms etc..

All the best.

Go to the File Menu, click on File, select Create, then Create Form.

It will open up the Prepare Form tool wizard, which will offer 3 options: Single File, Scanner, Create New.

Click on "Create New", then hit the blue button "Sart". After that, if you need to insert more blank pages into that new document, just use the Organzie Pages Tool, "Insert Pages" or open the thumbnail viewer in the left pane of the working space and click on the insert pages icon.

In any document processing application such as MS Word, and PDF making applications the pages would be generated as "Letter" format by default.

It is with this tool that you set the type of page format that you want for your document or just any particular page(s) in that document.

To turn your  document  page into an A4, you can just go to tools and select "Organize Pages", or even faster if you use press the "SHIFT+CTRL+P" keys and it will immediately open the "Set Page Boxes" functionality that you're looking for.

Srsly? It's 2021 and JS coding a workaround or a thrid-party tool is still the answer?

There's a neat create Form workaround with a few steps (from 2020) detailed here also:
https://community.adobe.com/t5/acrobat/how-to-create-a-blank-page-in-a4/td-p/11060082

Way easier to just build a bank of templates for your needs if you are stuck working with ProDC v2021.
Thankfully nowadays other systems & platforms (with UX/CX focus) support international users, teams and workflows.

Doh, shared this detail to the thread from whence it came.
Thanks for the Form workaround ls_rbls - sharing elsewhere for other A4 lost souls in the community.
