How to create /require a single selection in a row of checkboxes?

Greetings:

 

I'm trying to creating a web form for clients with a few feedback questions. Each question is allowed one selection in each row of 5 checkboxes. How do I create this? Or should I be using radio buttons or drop down menu?

I've use webforms several times but am having no luck with this idea.  I've attached a visual aid to help explain my goal.

I created this in MS Word, then saved it as a PDF. I use Adobe Acrobat Pro on Win 11. I can make whatever edits are needed. It's still in draft stage.

To do it you need to give all fields in the same group the same name, but unique export values.

A drop-down or a radio-button group will work too, of course. Although with a drop-down you will need to add a "neutral" selection (such as "Select") with no value, and with radio-buttons once a selection has been made the group can't be cleared, unless you clear the whole form (or use a separate button to clear that group's value).

Wow. That was fast.  I did experiement and figured out what I needed was radio buttons not check boxes to create mutually exclusive choices. So I made the edits and tested the web form and it was fine. All I will do is add the websform URL on another document and clients can fill out the feedback form at their leisure.
