Issues with template formatting for phone numbers and dates

I can't seem to get the formatting I selected for phone numbers and dates in the Text Field Properties format box to display accurately on my template. When I test out the template, phone numbers display as 0000000000 instead of (000) 000-0000. Same issue with dates. I tried locking the box after I make my selection, but that didn't do anything. What else can I try?

The Formatting option only works when you exit the field. And when you enter back into it it will revert to the original value. Is that what you've done?

What formats does you use? Does you use the template for Adobe Sign?

When you post a question you always need to tell the Adobe program you are using
There are MANY programs in a full subscription, plus other non-subscription programs

My apologies. I'm not a "tech" person, and my brain is already fried from the issue.

I'm using the Acrobat Pro DC. Hopefully that's the correct info needed to advance my question
