Need pdf creation help - auto-filling data from one page to another

Hi Guys,

 

I would appreciate any help.

 

I am trying to create a pdf for a rotation of activities and wish to create a pdf where I can fill the students names in once and it then auto-fills on all other pages.

 

I understand this might sound a little confusing so I have attached a video that shows along the lines of what I am trying to.

If fields have the same name, then they share the same value.

Ensure that you use a unique Field name for every specific field.

That'll ensure that every time you enter a value in a field it'll specifically be assigned to that field.

There'll be no duplication of values.

I am also making a form with multiple pages.  I have been doing do for years, and I swap in new pages when they need to be updated for some reason, but always use the appropriate field name so the pre-existing info will populate.

For some reason, this week, when I add a new page with the form fields, the entire doc loses all of its content within the fields, and never populates the new page either.  Thoughts?
