need some help with making sections for pdf

Hello everyone so i need help making a pdf but i have a drop down menu  where i want a certain form to apear for each option i have on the dropdown in the second section . and then i need for that sections to also appear filled out on the second section as well. please lmk if you arre able to help a guy out, at the bottom will be an example of what im looking to do its also for a website btw

Which Adobe application are you working with? The screenshots attached above don't look familiar. Are you designing a website and willing to upload a pdf form with the dropdown?
