PDF Form: Change automatic title of fields

I want to create a PDF Form out of a word document but I dont want the nearest text to be the name of a field. In the word.png you can see that under the line is a description if I create a PDF the description underneath the line is the name of a field but I want to have the text on the left to be the name of the input field.

Change the text under the field to the one you want to use as its title.

I know that, but I want to let everything happen auomaticly. So that everytime I create this PDF the field gets the correct name.

This is not possible.

In Acrobat, double click on the field to open up the properties. There you can change the name of the field.
