PDF not allowable as Fillable on all forms

I have created a couple forms for a client and on one form when I open it in Adobe Acrobat on Android the fields are not fillable.  I cannot tell the difference in how the file is saving to find out why it isn't working. I can also open two of the files in the Chrome Browser on my Mac and the form is fillable.  I'm unsure why the Beef Questionnaire is different and why the fields aren't working. They seem fine when I am in Acrobat Pro whilst creating the file.

If anyone has any ideas on what to check I would really be grateful.

There are no fillable form fields ont he Beef questionair.

@Thom Parker that's the weird thin.

Here I am attaching the files right after I saved them in Adobe Adrobat Pro.  When I open the files in Apple Preview on my Mac OS device they open right away and are editable.  Then I upload them to the website and they don't work.  Can you see if it works.
