Populate a required text field if a check box is clicked

I need help in finding the script for let's say, a yes and no option. If No is checked, the user can move along to the next form field. If Yes is selected, a text field either populates or it is activated and is required to be filled. Does such a thing exist? I cannot find it in previous posts unless I am not looking hard enough.

I forgot to add that, if the No and N/A check boxes are selected, it will skip that required (only if YES is selected) form field.
