Scan to a form template

I see walkthroughs on how to create a form from a scanned document, but I can't find anything on scanning into an existing form. I'd like to set up a form once from an initial scan, then what I scan from then on fills in form fields from scanned copy. Is this possible?

To clarify, the pdf form will always match the paper form for size, spacing, placement, etc...

Not possible with Adobe Acrobat.
