Text entered in fillable PDF disappears when I click to another field

I am using Adobe Acrobat Pro DC version 2020.013.20074

I have attached a screen recording of the issue.

I truly appreciate any help you might provide

The form fields work in the browser (Safari)

But, I cannot click the checkboxes unless I use Firefox

 

I am seeing where it might have gone wrong as when I right-clicked on it to open the form it opened in Preview (it did not give me an option to open in Adobe)

 

How can I save the form from the browser?

Do not open the file in any browser or in Preview!

Instead, right-click the link and select Save Target As (or something similar).

Then, once it is downloaded, open it in Acrobat directly.

It's most likely the result of the file being filled in using a non-compliant PDF application, like Apple Preview.

Stick with Adobe Acrobat/Reader in the future to avoid such issues.

For that file try exporting the data to an FDF or Text file and then importing it back in.

I downloaded it from a website and all form fields are blank

I added that text in Adobe to demonstrate what was happening

It's possible the file was edited in Preview before you got it. That would have corrupted it as well.
