None of those settings mean anything if the PDF is not opened in the Mobile Adobe Reader.
So in what app is this PDF being viewed?

it is being open in acrobat reader on the iphone at this point.
ill explain again:
first time was being open on whatever pdf opens in iphone. on that case the form does look like a form but is not alowed to be filled as phone reques password everytime u click on any area of form. i did explain the user it needs to be opened on acrobat reader and not on iphone default pdf reader.
once they downloaded the adobe reader for iphone, i verified myself and tried on my own phone, the for does open and does not require a password but form looks flat with no fillable forms, like the acrobad reader on the phone does not recognizes form as a form, but as a flat from with no fillable fields.
i works perfect on adroid, pc even on other pdf readers, but on iphone seems to not recognized the formats correctly on any pdf reader for iphone, even acrobar reader.

That's not good. Sounds like Acrobat for iphone has a bug. Have you tried the Readdle PDF Expert?

What app is being used on the iPhone to view the PDF? If its not the Adobe Acrobat Mobile Reader then there is no guarantee that the form will operate correctly.
And the type of password protection on your form is no real protection at all.
