Using Radio buttons in a fill-able form but it seems that you can only pick one at a time.

Example: I have multiple Yes or No Questions on this form and used a radio button for each.

I select one for the answer question one, when i go to question two and select an answer of yes or no, it wipes the answer to question one.

I am sure it is an easy fix, I just don't know what it could be.

Any help is much appreciated.

Give the radio button groups different names.
