Web Form - Split the download into multiple files

I have a web form that I send to patients to fill out new patient paperwork. This contains 4 different things I need and one they need. Is there a way to split the download so that when they've submitted it, the download file is split into File 1 (to be deleted), File 2, File 3, etc.  Ideally with specific names for each one?

 

Right now the process is to extract those pages which Adobe hasn't really figured out yet and is super tedious.  Panda sign has the option to do this.

What do you mean, Acrobat hasn't figured out how to do it? It can split files very easily, but not automatically when someone downloads them.

Maybe you should split them in advance and then merge them as a Portfolio, so you still have one file the user can download, but it has multiple other files attached to it, so they could submit them separately.

I mean automatically when someone downloads them. Portfolio is the right idea, but to be able to open all the files needing filling/signing with a single click, but maintain them as separate files when filled out. On Panda, there's a button for download as single file, or as multiple files.  Adobe has no such option. It'd be nice. The more steps I created for the patient (clicking multiple files) the rate of error goes MUCH higher. They need to be able to click one thing, fill it out, and ideally have it send it to me as multiple files like it was uploaded.

I don't see that happening. Having someone sign something they haven't opened or reviewed is a huge risk.

Also, what happens if not all files download correctly? Would it still allow you to sign them partially?

There's just too many risk factors in doing something like that, for a very small benefit.

They definitely still get all of the papers to review and sign, it's just saving back end work. 


So I have 4 files that go into a packet called new patient paperwork (NPPW).  There's the patient copy of the policies and procedures, I don't need that back. There's the NPPW which they fill out with all their data. There's the release of information to their primary care provider and the Card on File form for telehealth appointments. With Panda, I drop all of those into a single set that gets sent to the patient and when they fill it out, I can download them as one file or as 4 separate files, each with a signature authentication page.  How they ultimately get uploaded into my system is the latter. 


With Adobe, I have to put all of those files into one file and send it out to them for signatures.  Then it gets sent back and I can't immediately split it because it's signed so I have to print to PDF using Microsoft's PDF thing and then reopen the file. The split file option seems to just give me the option to do it by number of pages repeating.  So I go to organize pages and hit print page on the ROI form and save it as ROI and delete it from the file, then do the same thing with the COF form. Then delete out the patient copies. Then resave the file. Then upload the files separately. It's pretty tedious.  It'd be nice to have the option to download the files as separate groups. 

To my knowledge, I can't send a portfolio out with AdobeSign and if I make it so they have to click on multiple files, the drop out rate will be quite high.  We've tried that.

Please share some more details of what Panda sign can do and Adobe can't specific to your usecase. How exactly do you want to split it. Can you share the steps you are trying and the result you want?
