What happened to to "Customize Quick Tools" ?!

I'm no longer able to set up quick tools on the toolbar.  The only way to access the tools I need to use is to go though an annoying series of menu selections .  Did this recently change, or am I missing something?  I've scoured the entire program and came up with nothing, tried repairing the installation, no dice.  Below is a screenshot of what I'm seeing.  This is super frustrating!!!
