how do i delete double text when saving the pdf document?

When i try to save the fillable adobe form, it creates double text. I am really confused. How do i delete the double text or ghost or transparent text? Pleas help me soon. I am really lost.

    This is most likely caused by duplicate fields with the same name. There's nothing you can do about it using the free Reader.
