I have 40 pages PDF, want the header text link very page back to the First Page - Index. using Acrobat Pro DC. Any one had done this before?

It's easily done using a button field. Set it up on the first page, including the action, and then right-click it (in Form Edit mode) and select Duplicate Across Pages to create exact copies of it on the other pages of the file.

How to add text on the button. In the "Appearance" under the "Button Properties" has "Text" defined, but why doesn't show any text on the button surface?

It's the Label field under the Options tab of the Properties window.
