Make clickable hyperlinks from qr or zappar codes in pdf...how?

I have more pdfs with qr and zappar codes. Want to find way to make it clickable as hyperlink.

Any solution for this? Something like script or plugin which can "ocr" pdf and make clickable hyperlink over code.
