How do I use an existing fillable PDF form in wordpress? (e.g. email the form data or export it to a mySQL database?

I have an existing fillable PDF form. I uploaded it to the media library in wordpress. Once I link to the file and open it in a new window, the user can fill out the PDF form online. Is there a way to insert a submit button so the completed form data can be either emailed to me or saved in mySQL database using wordpress?
