IS THERE AN ADD ON TO USE WHEN THERE IS A FORM IN THE DOCUMENT BEING CONVERTED

I HAVE BEEN TRYING TO CONVERT A PDF DOCUMENT TO WORD. THE DOCUMENT CONTAINS A FORM. AFTER CONVERSION THE FORM IS NOT USEABLE. IS THERE ANY HELP FOR FORMS OUT THERE?

Are you trying to convert a PDF form or the form is attached to the document?

Please provide the exact workflow how you are trying to convert document to word file.
