Not all lines are converted in the same format using Export PDF

I am trying to convert a document into Excel.  95% of the lines come over in the same format (the information is in the correct columns and cells), but some of the lines have all of the information located in column A.  In looking at the original document, there does not appear to be any differences.

    Hi jamsw96,

    There have been some recent updates to ExportPDF, which should help with that. If you're converting from within Reader, please make sure that you have the most current version by choosing Help > Check for Updates. The current version of Reader is 11.0.09. And, if you're converting via the ExportPDF website, log out, clear the cache, and then log back in.

    If that doesn't help, please let me know. I would be happy to take a look at the file for you.

      Sara,

                      I am at the current version and I am still encountering the error.  I have attached the document that I am trying to convert.  I am also attaching the Excel document.
