I sent the pdf form by email.  It did not send and then my answers to the questionnaire was not on the form.  I can not find the saved e.all questionnaire at all

How exactly did you send the PDF? Did you use Adobe Reader for that or did you receive a document link in EchoSign email? If you are sending it through Adobe Reader, I would recommend to update it to latest version and then try:

Adobe Reader Install for all versions
