Buttons in Fillable Form linked to Captivate not working?

I have a .pdf form with checkboxes and two buttons - one button to clear the checkboxes and one to open another version of the form that is not fillable in another tab that is meant to be a printable copy.

The .pdf form works flawlessly on its own, separate from the Captivate project.

The final Captivate project has to be published in HTML5. When previewed or published in HTML5 the link for my forms described above react as follows: the button to clear the form always leaves at least one checkbox "checked" until pressed a second time, and the button to open the printable form in another tab does not open the other version of the form.

Any suggestions on how to fix this issue are greatly appreciated!
