I may switch from my Mac to a PC soon. What do I need to do to transfer my CC subscription?

There is nothing you need to do. For a CC subscription you are allowed to activate your license on up to 2 computers at a time so you could continue to use your Mac if you wanted. But if you are no longer going to use it you should deactivate the license on the Mac before installing on Windows so that you retain your 2 activations. https://helpx.adobe.com/download-install/kb/activate-deactivate-products.html
