Designing a form in InDesign and looking to create a header with a few text fields. When the user inputs the information on the first page, it will appear on all following pages. Please note, I am creating the interactive form by Exporting as Adobe PDF (Interactive). Hoping to achieve this in InDesign vs. preparing the form in Acrobat. Thanks in advance for any tips.

As long as the form field is named the same, it can be placed on multiple pages, and the input entered on page one will be entered on all subsequent forms/pages.

The area in which you can re-name a form field is shown in screen capture. You would select the bottom form field, and name it the same as the top form field.

Yes what you have shown above is great and that is exactly how I have done it. However, the fields do not work in sync..???

You will only see the form input once exported as PDF and viewed in Acrobat/Reader. Are you viewing in an application other than Acrobat/Reader ?

I just viewed my test in Acrobat Pro DC and it works as expected.
Do you want to share file via attaching it to an email ?

Looking at your files, I am seeing the same result, form fields names are appending: txt_branch_office1, txt_branch_office2, etc. And it appears to be a reported issue.
The workaround solutions that I can offer are either rename the form fields in Acrobat. Or, create Indesign document in CS6 where the form field names are not affected with this issue.

Just a silly guess...
are you sure you removed the space after the button name?
