When Saving new Files (or Save As) the option to embed color profile defaults to the OFF setting (unchecked). I don't recall this always being the default save setting. It's easy to forget to check this important setting, when saving multiple files in a production environment. The default should be ON, with the OPTION to disable. I checked PS prefs for an option to have current profile embedded by default with "Save As", but I see no option for that. (maybe I missed it).
I understand that when saving for web, other options maybe preferable, but that would seem to be the EXCEPTION not the logical option or rule. Perhaps I'm missing something?

I can't replicate that unless I create a document deliberately with no colour management.
Have you tried resetting prefs (the preference files contain more than the user set preferences). Back them up first though - that way you can restore easily if there is no change.

Did you try a "Save as"? I think that's actually where the issue lies.
OTOH, could be my prefs. But resetting prefs is such a poorly enabled option, I only do it when things are dire.
Thanks

In Save For Web, the "embed profile" checkbox is sticky. Once set, you shouldn't need to check it again. Mine has always stayed checked.
And yes, I agree with Dave. This sounds like corrupt preferences. Preferences are prone to corruption because they are rewritten on every application exit, as opposed to read-only program files. An irregular shutdown can easily corrupt prefs.
In any case, the quickest and best way to keep tabs on this:

@FauxtoGuy Yes I tried Save As and did not see the issue
