Make object look like it's coming out of another

Hi, I have this photo that I want the hand to look like its coming out of his stomach, can someone please tell me how I can achieve this? thanks!

A few ideas for you to work with:

Placed directly on the surface of the t-shirt, it looks like it doesn't belong

A few creases to distort the shirt

And the arm masked to make the creases appear to stand out

Probably the single most important thing, is a thin shadow between arm and shirt

And lastly, a weak shadow beneath the arm

Search on composition and masking tutorials. What you posted is crude. Masking is all about selections. Photoshop has many tools to help you select things well.
