Bars and Tones Audio not playing during playback.

Bleeping for curses and overwriting track 1 audio with tones. Sporadically the tones audio is not playing and instead is muted. All tracks are fine, nothing is muted or solo'd and all equipment is working properly. This is occuring over multiple projects and machines.

Check the Audio Hardware Preferences. Default Audio Input should be set to No Input.

Thanks Bob, but all preferences and settings are correct. It seems to be a bug or a system issue - which my best guess is clearing the media cache will solve.
