Looks like 'DogEars'. Try ctrl+shift+F11 (need to translate this to Mac lingo  )

I've never seen this before in Premiere, BUT if it is being put in there by Premiere then I would expect it's an overlay. Overlays can be removed by clicking that little wrench icon next to the 1/2 resolution dropdown and then just clicking "overlays" and they should turn off.
