Premiere Pro 25.2.3. MacOS 15.5, Macbook Pro 2021 M1.
After updating to latest OS, video transitions don't work. Has nothing to do with handles. Both third party and native transitions do not work.
I apply transition to the timeline. I see it in the timeline, but doesn't actually work. If I click on either clip on each side of edit, the transition icon disappears. Only standard dissolve works. Recently installed Red Giant Universe vfx and transitions as well. VFX plugins work fine.

@jimmy2cans
This sounds very frustrating.
I just double-checked on my 13-inch MacBook Pro M1 2020 and Transitions are working as expected in Premiere Pro 25.2.3 under macOS Sonoma 14.7.5.
If you check Settings > Timeline > Video Transition Default Duration, what is that set to?
If you download and open one of the Learn projects (like the coffee shop footage), do Transitions work as expected?
As you trouble-shoot this, I'd take the time to temporarily disable third party plugins via the Reset Options. Resetting the Preferences, clearing the Media Cache, and clearing the plug-in cache may also be helpful.

Does it play OK if you press Enter to render the timeline?

No. It's strange. I apply the transition. In the timeline the transition icon or whatever appears. Transition doesn't work. And, if I click on a clip on one or the other side, the transition icon disappears. Very weird.
