Name field missing when send PDF to recipient

I try and add a field - Name and Company to be filled in by the recipient. When i send a test document to the recipient the name field does not appear, but all the others do. Any ideas?

Please advise.

    I see that you have Adobe PDF Pack subscription.

    As you said that you added the fields to form, please let us know how you have created the form.

    Please share the exact steps you do to create and send the file.

    If possible, can you share the original file and the file on which you have added the fields?

    I am having the same issue...

    I uploaded a preexisting PDF to Adobe Sign and added fields in that program.

    The template in my library shows all fields, but when I send a test document to sign, all "Name" fields are missing.

    Please advise.

    Could you please let us know if this issue occurs with a particular PDF form.

    Have you tried to replicate the issue with other forms? If not, please check with other forms.

    Let us know what happens.

    Can you share the form with us, so we can check it on our end?
