Radio Button Unchecking When Uploading to Adobe Sign

In Adobe Acrobat, I'm filling out a form that contains text fields, check boxes, and radio buttons. When I use Adobe Sign via the "Collect Signatures" option, one of the radio buttons that I had checked in Acrobat is now blank.  All the other data seems to be fine.     

On the attachment, its the radio box on page 3, tab 2 (Probation Term).  Its is duplicated on page 4, tab 16.
