Separate signature fields required in a web form

For example: this is the issue I'm having shown in this picture.

I have it so that if a client is between the ages of 14-17 that a signature field will display that requires a signature of a parent but I can't seem to make it so that it is a separate signature. It auto populates the same signature for the client and parent.
