FormMaker Pro looks like exactly what you want.
I always do all of my forms work in Acrobat, so when I need to adjust layout in InDesign I simply make the change, export a fresh print PDF, and then go back to my interactive PDF in Acrobat and Replace All Pages. I've make an AutoHotkey script that lets me do all of this with a single keystroke.
However, I'm looking at FormMaker Pro right now, and wondering if it would save me $300/yr of time.
